There’s an app for that. Jonathan Benson, one of the “founders” of the CMI at NMSU, among other former local positions, including work for local politicians, is now working at something called DPS Health out of LA.
DPS Health being one example of coming move to DIY healthcare using cloud tools. They promote something they call “Virtual Lifestyle Management”, and have a “Behavior Change Suite”.
